Overview
Clinical Steps
1. Contour Maxillary & Mandibular Rims
- Maxillary: contour to the patient’s interpupillary line, resting lip position, and Camper’s Plane
- Mandibular: contour to the appropriate lip prominence + position relative to the maxillary wax rim
2. Establish Vertical Dimension
- Determine & make appropriate Record based on the patient’s occlusion

3. Mark Occlusal Rims
- Use buffalo knife to create scores in the wax material
- Mark the following:
- Midline
- Canines (Labial Commissures)
- Smile Line
4. Tooth Selection
- Ask for the BlueLine book from the dispensary
- Shade match the patient’s existing teeth (if applicable)
- Determine the appropriate width of the denture teeth based on remaining width
- Allow patient to help select the shape and color of the teeth
- ** Make sure to write down shape & color of chosen teeth in your Axium note!

Findings and Treatment:
Maxillary record base was first adjusted to be parallel with interpupillary line and Camper’s Plane and 1 – 2 mm below resting lip line. Speaking sounds were used to confirm adequate positioning of the maxillary occlusal rim. Mandibular rim then contoured to appropriate lower lip contour and horizontal positioning compared to maxillary rim.
Vertical dimension was assessed and determined using a tongue blade. VDO determined by subtracting 2 mm from patient’s rest position.
Midline, canine, and smile lines marked on wax rim. The inter-canine distance was ___ mm. Teeth in shape ___ and shade ___ were selected.
Centric Relation / Maximal Intercuspal record taken using Bite Registration / Aluwax. Facebow of maxillary occlusal rim also obtained.
Post-Op Instructions / Limitations: __
Evaluation: Patient was cooperative and tolerated treatment well.
Next Visit: Wax try-in of dentures. Patient was asked to bring a family member or friend to the next appointment to help evaluate the esthetics of planned dentures.
